Everyone gets tired from time to time, but if you feel exhausted day after day, it may be that you are suffering from fatigue. By answering our simple questionnaire you could help your doctor assess whether or not you are experiencing the symptoms of fatigue. The questions should take you at most three minutes2 to complete.

How to use this questionnaire
Doctors around the world use the FACIT-F questionnaire to assess fatigue levels. Your answers will help you and your doctor to understand your experiences of fatigue and how fatigue may be affecting your life.

Some helpful hints:

  • Each of the questions asks you to rate how you feel on a scale.
  • You should read each question carefully as some of the questions are positive, e.g. “I have energy”, and others are negative, e.g. “I feel tired”.
  • Based on your answers to these questions, we calculate a total fatigue score that you can discuss with your doctor.

Your privacy
We do not ask for your personal details in the fatigue questionnaire and your answers will not be recorded on our systems. If you would prefer not to enter your information online, you can print the questionnaire for your own use.

Your fatigue score is calculated as the total score from the answers you gave. A higher score can indicate that you are feeling better and are less fatigued. 52 is the highest possible score and suggests the least amount of fatigue. Zero is the lowest possible score.

The fatigue score is calculated as follows:
For all questions, apart from numbers 7 & 8, we invert the score shown on the questionnaire, such that 4 (Very Much) is scored as zero points and 0 (Not at all) is scored as four points. We total up the scores to give a number out of 52, where a score of 52 suggests you are least fatigued.

Remember That a score of 52 suggests the least fatigue, and a score of 0 the most fatigue.

Talking to your doctor about the results

The questionnaire is designed to help you assess your level of fatigue and to help you explain to your doctor how fatigue is affecting your life. You can print the results of the questionnaire here.

Whether the score assessed by this questionnaire is high, low or somewhere in the middle, if you are concerned about how tired you are feeling it is important for you to discuss this with your doctor. Your doctor is the best person to fully assess your level of fatigue and what the underlying cause might be. When you visit your doctor it is also important to mention any particular issues or long-term medical conditions that may be contributing to your tiredness.

Tracking your fatigue each week
You can track how your level of fatigue changes over time by taking this 3-minute questionnaire once a week. This might help you to assess how your fatigue relates to events in your life, and may help your doctor to better understand what could be causing your fatigue.
